You will need to obtain a Doctors RX from your child’s Pediatrician prior to the Evaluation. The Doctor’s Prescription should state the recommended Evaluation e.g. Occupational Therapy Evaluation, Physical Therapy Evaluation or Speech Therapy Evaluation. A separate RX is needed for each discipline the child is being evaluated.
The RX must include Date, Child full name, address, date of birth, diagnosis, and type of evaluation the child is recommended to receive e.g. (OT,PT or Speech).
A Comprehensive Occupational Therapy, Physical Therapy and Speech Evaluation’s include Standardized assessments, clinical observations, parent interview and questionnaire, and a teacher questionnaire when agreed upon.
At times a school visit is indicated and scheduled when permitted at an additional fee.
You will be given a Health Insurance Claim Form with the Insurance codes so you may submit to your insurance carrier for reimbursement. A Diagnosis is needed in order for any insurance carrier to process the claim.
A non-refundable deposit of $100 is required at the time evaluation is scheduled. The remainder of the balance is due at the time of the evaluation.
The evaluation can take up to 2 hours so prepare for a 2 hour block of time in your schedule.
You are entitled to an evaluation either through the Early Intervention Program for children (0-3 years) or the Department of Education (3 years and above) at no cost to you.

We accept EI, CPSE, CSE and private pay. We do not directly work with insurance companies and are considered an out-of-network program. Some insurance companies may offer reimbursement for the services we provide and we recommend checking with your insurance carrier directly.
